Please fill out your information and choices of babysitting dates as accurately as possible. Forms must be completed and submitted by September 25th. You must complete the schedule and billing portions completely and accurately ASAP. High Holiday Babysitting will be held in Rooms 5 & 6. 

Please double check your form before you submit.  Inaccurate information or an incomplete submission will result in failure to enroll in babysitting for the High Holidays.
